Job Overview: Provide case management to children with serious emotional disorders, and their families to ensure access to culturally appropriate resources required for a stable, safe and healthy community life.

Job Duties and Responsibilities:

  • Caseload 20-25 per child worker
  • Provide consumer contact bi-weekly or monthly as designated by MBH's environment matrix updated every six months
  • Provide assistance to consumers to identify, access, and learn to use community resources
  • Provide initial intake and referral
  • Conduct strength based assessments, within thirty days of registration, annually thereafter
  • Conduct service planning within thirty days of registration, every three months thereafter
  • Assist in developing informal network supports
  • Provide crisis interventions
  • Attend trainings as required
  • Interact with appropriate funding sources
  • Maintain accurate record keeping and documentation of all contacts
  • Assist in identifying gaps in service for purpose of program development
  • Perform all other duties as required by the supervisor

Job Qualifications

  • A Bachelor's Degree with major course work in sociology social work, psychology, gerontology, criminal justice, theology, nursing, counseling or education.
  • Be a registered nurse
  • A High School Diploma and 12 semester credit hours in field of study previously identified and 2 years' experience in public or private human services with 1 year in direct client contact.
  • High School Diploma and 5 years of mental health direct care experience in public or private human services with employment as an ICM staff person prior to April 1, 1989
  • Must have a valid Driver's License
