CHDC EXPANDS BEHAVIORAL HEALTH DEPARTMENT - INCREASING ACCESS TO MENTAL HEALTH SUPPORT
Pottstown –Community Health & Dental Care (CHDC) was awarded a grant from Heath Resource Service Administration (HRSA) to expand behavioral health support – proudly offering comprehensive behavioral health and psychiatry services to fully support overall well-being health needs – from kids to adults! As the region’s leading Federally Qualified Health Center (FQHC), CHDC currently provides care to over 29,000 registered patients throughout southeastern Berks, western Montgomery, and northern Chester counties with locations in Pottstown, Barto, and Boyertown.
CHDC’s Total Care Program supports more than improving patients’ physical health but looks after their mental health as well by offering a fully integrated behavioral health and medical program. Equipped with an incredible team of behavioral health consultants and psychiatry providers, the CHDC mental health team works with its caring primary care providers to offer one-on-one appointments for patients of all ages providing patient-centered mental health support at all locations within person and virtual visits available. Individuals struggling with depression, substance abuse, family and relationship issues, grief and loss, or school and work stress, CHDC’s expanded behavioral health support is now available to all CHDC established medial patients.

For psychiatry, we are pleased to announce CHDC medical provider, Collette Hain, MSN, CRNP, PMHNP-BC, FNP-BC
is now a Board-Certified Psychiatric Mental Health Nurse Practitioner who will see patients of all ages – kids through adults – onsite at our main Pottstown health center location or virtually. Psychiatry appointments are scheduled through an internal referral and after a CHDC Behavioral Health and CHDC medical provider consult has been met.
For behavioral health, we welcomed our newest Behavioral Health provider, Laura Galindez, LCSW, to our main health center at The Shoppes at Coventry in Pottstown. Laura is committed to forging authentic connections with patients from all backgrounds, and is an experienced psychotherapist, social worker, and health coach. Laura joins Andre Hewitt, an experienced behavioral health provider working with children and adults who are facing a wide range of mental and emotional difficulties such as anxiety, depression, anger, and impulse control.

For qualified patients receiving a 100% healthcare discount, the nominal fee for a medical/behavioral health/vision visit is $25, $40 for a dental visit and $7 for a prescription! CHDC can offer the health care discount because we receive annual funding from our Federal partner, Health Resources and Services Administration (HRSA) since 2012 as a designated Federally Qualified Health Center.
